M-Compass Application Requirements

Below are some key documents that apply to all students who apply for any U-M program in M-Compass. The purpose of this page is to provide students with a preview of these documents and requirements. The School, College, or Unit sponsoring the M-Compass program may have additional, program specific requirements. To apply for a program, students must complete all requirements in M-Compass.

I have read and understand the information associated with this funding application.  I affirm that the information I have provided on this application form and any additional material I submit is complete, accurate and true.   I understand that furnishing false information or failure to provide complete or updated information may result in revocation of any award.  I authorize the University of Michigan to release this scholarship application as well as my academic, demographic and financial information to program administrators in order to determine my funding eligibility. The University of Michigan offices, departments or programs providing information and receiving student record information will do so consistent with the Family Education Rights and Privacy Act (FERPA).

By signing below, I agree to the following terms and conditions of the university related travel experience:

Travel Risks and Waiver

  1. I understand that I am not required to participate in this travel experience. The decision to participate is completely voluntary.
  2. I agree to waive and release the Regents of the University of Michigan, its employees and agents (hereinafter “U-M”) from any claim arising while participating in the travel experience, whether for injury, loss, damage or expense resulting from illness, quarantine, accident, crime, natural disaster, terrorism, act of war, or government restrictions and regulations, provided that such claim is not due to the negligence of the released parties. For U-M managed programs, I agree to waive and release the program administrators, host school/organization, and all of their employees and agents from the same claims.
  3. I understand that participation in this travel experience involves risks. I recognize that I may be exposed to potential risks including, but not limited to, accident, war, natural disaster, sickness, quarantine, terrorism, serious bodily injury or death, crime, property damage and other risks that may not be foreseeable. I understand that the university cannot guarantee my health and safety. I have investigated the risks, including reviewing the U.S. Department of State travel advisories, CDC Travel Health Notices, and U-M Travel Designations as listed on Global Michigan, and I assume responsibility for any and all risks associated with my travel.
  4. I recognize that the U-M does not represent or act as an agent for, and cannot control the acts or omissions of any non-U-M program administrator, host family, employer, transportation carrier, hotel, tour organizer or other provider of food, goods, or services involved with the travel experience. I understand that the U-M is not responsible for matters that are beyond its control, and that it cannot warrant the safety or convenience of the circumstances under which I will be living or working.
  5. When traveling internationally, I understand that I am subject to host country laws. It is my responsibility to familiarize myself with all laws, rules, orders, regulations, and the like that may be applicable to my travel, including, but not limited to, the host country laws, rules and regulations, local public health guidelines, health testing/immunization requirements, and the import of over the counter and prescription medications. I also understand that I must continue to follow all applicable standards of U.S. law and U-M academic standards including, but not limited to, research guidelines and export control requirements.

Financial Terms

  1. I accept responsibility for any tuition, fees, travel and other expenses that may be associated with the travel experience. For U-M managed programs, I understand that the program organizer reserves the right to adjust fees if exchange rates affect program logistics and/or local expenses.
  2. I understand that the U-M and/or any external program organizer reserves the right to alter program arrangements or cancel any program as conditions warrant and that I will be responsible for any associated costs that may result. These costs may include, but are not limited to, unanticipated flight changes, temporary accommodation charges or fees, and non-recoverable costs for any portion of the experience.
  3. I agree to indemnify U-M and all of its employees and agents from any financial obligations or liabilities that I may personally incur while participating in this travel experience, including attorney’s fees and court costs resulting from my acts, errors or omissions. I agree to accept responsibility for any damages that I may cause to facilities, housing, or other property during the travel experience.

Personal Conduct and Behavior

  1. I agree to conform, during the travel experience, to all reasonable standards of conduct, local health guidelines or orders, and public health or emergency response protocols, including those provided by any program administrators or host organizations.  If I fail to maintain any of those standards, I will be subject to disciplinary action, up to and including the termination of my participation in the experience.  I accept personal responsibility for transportation costs to return home.  I will forfeit any fees or other expenses incurred for the experience. For students only: I will also follow the guidelines set forth in the following document that is relevant to my enrollment status:

Health and Safety

  1. I agree to comply with any U-M requests for travel health planning, which may include reviewing the U-M Travel Health Preparation Guide and/or completing the Travel Health Questionnaire & Verification in M-Compass. I understand that I am responsible for managing my own health before, during, and after the travel experience, including any pre-existing conditions or health vulnerabilities.
  2. I understand that if U-M learns I am experiencing serious health problems, have suffered an injury, or am otherwise in a situation that raises significant health and safety concerns during the travel experience, then U-M may contact any person who I have provided as my “emergency contact” or any family members known to U-M.  I understand that the U-M does not ordinarily initiate such contact without first having a discussion with me.
  3. I agree to the following U-M recommended risk management precautions:  While operation of a motor vehicle is strongly discouraged, I will consider obtaining liability and collision insurance that will cover me in the applicable foreign countries if needed.  I will consider purchasing tuition insurance (if applicable) and travel insurance to protect my personal property from loss or theft.
  4. I agree to comply with all University and governmental health requirements or orders for participating in the program and for returning to campus after the program concludes, including immunizations, testing, quarantine, and other local health regulations or directives.

Potential Travel Disruptions

  1. I understand that events outside of U-M’s control may disrupt the travel experience, and I agree to follow U-M directives when such events occur. Such events include, but are not limited to, political unrest; natural disasters; airport closures; changing immigration and entry/exit requirements; increases in U.S. Department of State Travel Advisory levels, CDC Travel Health Notices, and/or U-M Designations; and changing laws, regulations, and orders, including public health regulations at the travel destination(s). I understand that I am responsible for any costs associated with travel disruptions.

Additional Health and Safety Terms for International Travel

  1. I agree to maintain required U-M travel abroad health insurance for the duration of my international experience (either by individually purchasing coverage or by confirming included coverage with my program organizer).  I accept responsibility for obtaining required immunizations and researching health issues in my host location. I agree to comply with the U-M International Travel Policy SPG 601.31, including following requirements for locations under U-M Travel Warnings, Travel Restrictions, Travel Health Medium Risk, or Travel Health High Risk and maintaining accurate travel registration throughout the duration of my experience.
  2. I will consider obtaining U-M Travel Abroad Health Insurance for any personal/leisure travel before or after my international experience.
  3. I understand that it is my responsibility to understand the coverage details of the U-M Travel Abroad Health Insurance and that any cost not covered by the U-M Travel Abroad Health Insurance is my responsibility.
  4. I understand that there is a possibility that I will not be able to leave the travel destination as planned due to transportation disruptions or changes in entry/exit immigration regulations and that I should be ready to shelter in place as, and if, necessary. Sheltering in place includes securing food, water, and shelter; verifying access to funds; and developing a local support network.

Additional Terms for U-M Managed Student Programs

  1. For U-M managed programs, I agree to inform official representative/s of the program or the host institution of my personal travel plans during free periods. I accept full responsibility for my activities during independent travel and during absence from supervised program activities.
  2. In a medical emergency, if I am unable to give consent, I give permission to the program leaders to authorize on my behalf medical treatment by licensed medical personnel; this includes authorization to hospitalize, secure proper treatment, administer anesthesia, conduct x-rays, or perform surgery.  I understand that this authorization is given in advance of any specific diagnosis, treatment, or medical care being required and is to serve as specific consent to any and all such diagnoses, treatment, or hospital care that is recommended or deemed advisable.  When requested, I agree to make repayment of any expenses advanced.
  3. I accept responsibility for requesting reasonable accommodations related to a disability in a reasonable time frame before departure.  I understand that I must provide the university’s Services for Students with Disabilities office with documentation of my disability to be considered for accommodations.  I further understand that I am responsible for coordinating requested accommodations and that my requested accommodations may not be available at the travel destination.
  4. I understand that program organizers and host institutions have the authority and right to cancel programs for health, safety or security reasons. In the event that I am en route to or at the program site during such a cancellation, I agree to depart the program site or cancel my planned arrival at the site, and return to the location designated by the U-M immediately.
  5. I understand that I am responsible for all expenses related to the program, including any unforeseen expenses that may not be listed in my U-M program budget or cost sheet.
  6. I understand that there are circumstances under which program organizers and host institutions have the authority to withdraw students from the program. Students withdrawn or dismissed from the program will not receive credit and will not be entitled to any refund. These circumstances include but are not limited to the following:
    • Students whose conduct before or during the program is deemed undesirable and whose actions are in violation of the following document that is relevant to their enrollment status: U-M Ann Arbor Statement of Student Rights and ResponsibilitiesU-M Dearborn Student Rights and Code of Conduct, or U-M Flint Student Code of Conduct.
    • Students who purchase, possess, and/or use any illegal or unauthorized drugs during the duration of the program, including free time.  This includes drugs that are illegal in the United States and/or of the country of participation.
    • Students whose conduct is in violation of the rules and guidelines of the partner/host institution.
    • Students whose conduct is in violation of regulations set forth by the on-site Resident or Program Director.
    • Students whose conduct is in violation of the host country’s civil and/or criminal statutes.
    • Situations in which the student’s behavior, whether academic or social, jeopardizes the reputation of the program, the participation of others in the program, or the health and safety of the student or other participants.
    • Failure to participate in the required pre-departure, on-site, and post-travel orientations, programs and assignments.
    • Failure to maintain application requirements (academic or disciplinary), including course prerequisites, or failure to notify program organizer of any enrollment status changes.

7. I understand that if I choose to withdraw from the program for any reason, I am responsible for any and all associated costs and am subject to the academic policies of the U-M and host institution.

This information is for students considering disability accommodation options abroad.  Managing your disability abroad can be a significant source of growth, challenge, and reflection in a new and unfamiliar environment. You may already have accommodations on campus, but there may be different factors to consider in your host community and culture, especially since the disability infrastructure and support options are not standardized around the world. It is important to research the country/region you are traveling to in order to determine a good fit for your disability and accommodation needs.

We encourage you to research options early and before committing to a program to determine which location and program are the best fits for your needs. Not all accommodations are possible in all locations or for all programs. Some considerations are outlined on the Thinking Through Traveling Abroad: Questions for Students with Disabilities to Consider document and on the Global Michigan Disabilities Abroad page.

I understand that, during the course of my participation in this program, the University, or those acting with the University’s permission or authority, may capture my name, likeness, image, and/or voice in photographic, audio, video, digital or other forms (“Recordings”).  I recognize that the University, through its Board of Regents, holds the copyright in all Recordings.  In addition, I, hereby, permit the University and those acting with its permission or authority to use my name and/or the Recordings, in any and all media (including the Internet) now existing or hereafter devised, for any purpose consistent with its mission.  I understand that I will not have an opportunity to review or approve uses of the Recordings.

I permit them to use my image in promotional materials and to adapt such photographs as necessary for space and purpose constraints. I also understand that I will not receive payment or any other compensation in connection with the pictures and recordings.

I, hereby, release and discharge the University, its agents, and legal representatives from all claims and demands by me, my heirs, or assigns arising out of or in connection with the taking and/or use without limitation and from all claims which may or could arise from the taking or use of the Recordings.

I understand that if I do not digitally sign this form, I must inform the Program Sponsor to make alternative arrangements and the Program Sponsor will mark this requirement as complete.

You indicated that you have been under a healthcare provider or specialist’s care for an ongoing/recurrent psychological or physical condition within the last five years, that you have a life-threatening allergy, or that you will be taking prescription medications during travel.

Next Steps

  1. Download the Travel Health Preparation Guide.
  2. Bring a printed or digital copy of the guide with you to see a professional healthcare provider (e.g. private physician, counselor, University Health Counseling, etc.) to discuss your conditions and to develop a self-care plan.
  3. Return to M-Compass on a later date to sign this document. Do not sign until after you have consulted with a healthcare provider.
  4. After seeing your provider and discussing your self-care plan, sign below to verify completion.
  5. It is my responsibility to disclose health conditions or needs to individuals where appropriate since the University of Michigan will not collect my self-care plan or any additional information about my personal health, allergies, or medications. Examples of who to disclose my information to may include travel companions, roommates, program leaders, or on-site colleagues who should be aware of my health conditions, medications, or allergies in order to assist in case of emergency and/or help with local arrangements to meet my needs.

I, {FIRST NAME} {LAST NAME}, verify that I have reviewed the Travel Health Preparation Guide with a professional healthcare provider and developed a self-care plan for my travel experience.

I understand that the University of Michigan (“University”) has issued a U-M higher risk travel designation (University Travel Warning, University Travel Restriction or Travel Health Medium or High Risk designation) for my travel location because it has determined that there is a significant level of risk to the health, safety, and/or security of University personnel, students, property, or other interests in this location at this time.

I am aware of the U-M higher risk travel designation for this location and nevertheless desire to be in this location, which may be for an indefinite period of time.  As a condition of my being allowed to engage in University of Michigan Related Travel at this location, I agree to comply with the University’s International Travel Policy SPG 601.31 as the same may apply to me during my stay (as all University personnel and students must do), and additionally agree as follows, as the following may highlight the requirements of SPG 601.31 or supplement the same:

  • I will follow the U-M eligibility and requirements for U-M Related Travel as outlined on my destination/s’ Country Risk Rating page/s on Global Michigan.
  • I will maintain accurate travel registration, including trip itinerary and on-site contact information, at all times.
  • I confirm that I am enrolled in, and at all times will remain enrolled in, the University-approved travel abroad health insurance coverage.
  • I understand that I am responsible for researching and understanding the risks associated with my location and for developing appropriate mitigation strategies, as the same may need to be modified during my stay.
  • I recognize that the University cannot guarantee my safety, and I acknowledge that my decision to travel to a location under a U-M higher risk designation may expose me to significant physical, health, or other risks. I understand that I am responsible for my safety and I assume responsibility for all risks associated with my decision to remain in this location.
  • I voluntarily agree to be at this location, which is subject to a University of Michigan Travel Warning or Restriction or Travel Health Medium or High Risk designation.

Additionally, I, for myself, my heirs, personal representatives or assigns, do hereby agree to indemnify, release, waive, discharge, and covenant not to sue The Regents of the University of Michigan, its officers, employees, and agents (the “indemnified parties”) from liability for any and all claims related to or arising out of my decision to engage in University of Michigan Related Travel at this location, including, but not limited to, those that result from the negligence of the indemnified parties, and regardless of whether the same are for injury, loss, damage or expense resulting from accident, war, natural disaster, pandemic, epidemic, crime, sickness, quarantine, terrorism, government restrictions and regulations, or any other cause.

For the avoidance of doubt, the waivers, assumptions of risk and indemnities provided by me in this waiver are in an addition to, and not in lieu of, any protections, defenses or other remedies or immunities afforded to The Regents of the University of Michigan, and its members, officers, agents, employees, and any other persons or entities acting on their behalf, by Michigan law.

I agree that enforcement of this agreement will be under the laws of the State of Michigan, should any dispute arise.

Travel to a country where Crisis24 Political and Natural Disaster Evacuation Coverage Is Unavailable

I understand that the University of Michigan (“U-M”) provides political and natural disaster evacuation assistance through Crisis24, the U-M’s political and natural disaster evacuation assistance provider, which is contracted through the GeoBlue (BCBS Global) Travel Abroad Health Insurance program (“U-M Travel Abroad Health Insurance”).

I understand that, due to current conditions, Crisis24 will not approve political and natural disaster evacuation coverage for the foreseeable future for certain destinations. See “Locations with No Evacuation Coverage for Future Travelers” on Global Michigan’s Crisis24 page.

I understand that my destination has no political and natural disaster evacuation coverage for the foreseeable future and that U-M will not be able to evacuate me without this insurance coverage.

I understand that conditions in a location that lacks Crisis24 political and natural disaster insurance coverage present a significant level of risk to my health, safety, and/or security, and that conditions may change rapidly due to factors that can include political instability, armed conflict, terrorism, civil unrest, government actions or restrictions, infrastructure disruption, natural disasters, or public health events. I understand these conditions may limit or prevent my ability to depart at a later time, including due to delayed or canceled flights, border closures, curfews, or dangerous conditions related to travel within the country/region (including travel to an airport). In the event of such a political or natural disaster event, I will be responsible for leaving the country using my own resources (including funding) if I choose to do so. Without Crisis24 political and natural disaster evacuation coverage, U-M can only provide guidance in the event I choose to leave the country.

Acknowledgment of No Evacuation Coverage / No Evacuation by U-M

I acknowledge and agree that because my destination has no Crisis24 political and natural disaster insurance coverage:

  1. Crisis24 political and natural disaster evacuation coverage is not available/approved for my destination for the foreseeable future, and I will not have access to that coverage for this travel.
  2. I cannot be evacuated by U-M or Crisis24 from this destination.
  3. Neither U-M nor Crisis24 will cover or reimburse any costs associated with departure from the destination, including any later attempts to depart, or having to stay at the location due to an inability to depart.

Assumption of Risk and Conditions of Participation

I am aware of these risks and nevertheless desire to travel to and/or remain in this destination, which may be for an indefinite period of time. As a condition of being allowed to engage in U-M related travel to this destination without Crisis24 political and natural disaster evacuation coverage, I agree to comply with the following:

  • No guarantee of safety. I recognize that U-M cannot guarantee my safety or security. I acknowledge that my decision may expose me to significant physical, mental, health, legal, financial, or other risks.
  • Personal responsibility. I understand that I am responsible for my own safety, security, and decision-making while in the destination, including arranging any assistance, transportation, lodging, communications, and contingency planning as needed.
  • Travel at my own risk. I am responsible for travel within the destination (including travel to an airport, border crossing, or other departure point) at my own risk.
  • Financial responsibility. I am responsible for all costs associated with traveling to, remaining in, or leaving the destination, including but not limited to costs for lodging, transportation, rebooking flights, independently arranged evacuation or extraction, and costs arising from delays, cancellations, or changes in conditions. Crisis24 and U-M will not reimburse these costs.
  • Academic/employment consequences. I recognize that conditions may prevent me from returning to campus or my assigned work location as planned. If I am unable to return for an academic term, appointment, or other U-M obligation, I accept full responsibility for any academic, employment, financial, or other consequences of my decision.
  • Voluntary choice. I voluntarily choose to travel to and/or remain in this destination despite the categorical unavailability of Crisis24 political and natural disaster evacuation coverage.

Release, Waiver, and Indemnification

Additionally, I, for myself, my heirs, personal representatives or assigns, do hereby agree to indemnify, release, waive, discharge, and covenant not to sue The Regents of the University of Michigan, its officers, employees, and agents (the “indemnified parties”) from liability for any and all claims related to or arising out of my decision to travel to a country where Crisis24 Political and Natural Disaster Evacuation Coverage Is Unavailable including, but not limited to, those that result from the negligence of the indemnified parties, and regardless of whether the same are for injury, loss, damage or expense resulting from accident, war, natural disaster, pandemic, epidemic, crime, sickness, quarantine, terrorism, government restrictions and regulations, or any other cause.
